The parties to this Agreement, The Regents of (college) on behalf of (campus site) hereinafter called (name) and the Contractor, whose name and address are set out immediately above, hereinafter called 'Contractor', agree as follows:

1) Scope of Service: Contractor agrees to provide sign language interpreting, transliteration, or captioning services as defined by (name of policy) and (name of student services disability office) according to his/her skill level and ability to provide "qualified" interpreting/captioning, as defined by the Americans with Disabilities Act as it relates to student academics, to Deaf and hard of hearing students at (college) for mutually agreed upon hours. Arrangements for interpreting/captioning services at (college) not facilitated or agreed upon by (disability office) do not fall under the scope of this agreement (such as programs, events, extra-curricular activities, etc. which are non-academic and the responsibility of each individual (college) department.

Other services to be provided as agreed upon: Contractor will provide interpreting services for student (name) in (country) for (dates) excluding (i.e. holidays). Interpreting services will be for classroom time, as well as for extra-curricular activities/meetings etc. up to (number of hours) per week.

5. Compensation: University agrees to pay Contractor as full compensation for the service described above at the rate of (amount) per month, less applicable state and federal taxes.

Other rates or conditions of compensation include: Payment for salary and other expenses shall not exceed  (amount). No extra payments will be made for salary, airfare, housing, insurance or any other expense beyond the (ending date).

Payment shall be made to (name) and mailed to (address).

Equipment/Materials to be provided by Contractor: Contractor agrees to provide at his/her/their own expense the following equipment, information, and materials: (i.e. airfare, housing, food).

Further Conditions: Any other stipulations agreed upon must be clearly and specifically documented herein: (i.e. In the event that the assignment is terminated before the contract has been completed, due to the student or interpreter ending the program unexpectedly for any ethical or emergency concerns, payment will be prorated to the time of employment already served.)

In addition, Contractor hereby agrees to each of the following:

a) Termination: (College) shall have the absolute right to terminate any Service/Event at any time when, in the sole opinion of (college), continuing such Service/Event shall constitute a risk to the health or safety of event participants or members of the campus community.

b) Independent Contractor: Contractor shall conduct the Service as an independent contractor and not as an employee or agent of (college). As an independent contractor, Contractor shall have no entitlement to (college) workers' compensation benefits.

c) Hold Harmless: Contractor is fully aware of and understands all of the risks which may be associated with the Service and voluntarily assumes any and all such risks. Contractor agrees that (college) shall not be liable to Contractor for any injuries incurred by Contractor in connection with the Service or for any loss of, or damage to, Contractor's property, unless such loss is caused by the sole negligence or willful misconduct of (college), in its agents or employees.

d) Indemnification: Contractor hereby agrees to defend, indemnify, and hold the Regents of the University (name), its officers, agents, and employees harmless from and against any and all claims, demands, judgments, costs and expenses arising out of the negligence or willful misconduct of Contractor in conducting the Service. Contractor agrees to reimburse (college) for the actual cost of repairing or replacing any (college) equipment or property damaged by Contractor while Contractor is on (college) premises or (overseas site) premises. If legal action is required by (college) to enforce any of its rights under this Agreement, Contractor agrees to reimburse (college) for reasonable attorney's fees and court costs.
